- Please come on time for your appointment. If you come 15 minutes earlier or later than the scheduled appointment, you'll be on the walk-in waiting list. We encourage you to schedule the visit for prompt encounter. We do not do any refill medication during walk-in visit.
- LEP (Limited English Proficiency) policy. We encourage patient to call their insurance/ Federal-state provider to help in language/ communication barrier before having the visit. We provide the information during the visit (it may take time, so we encourage to do it before the visit) to get language access help through federal-state provider or your insurance.
- NO SHOW POLICY : a patient is considered NO SHOW when a patient fails to keep a schedule appointment or reschedule within 48 hours. After 3 NO SHOWS within 6 months, you will be discharged from our facility or will be charged $10 for no show penalty on the visit, and applied onwards for future NO SHOWS.
- Waiting time. Walk-ins will require you to wait at a longer time to accommodate scheduled patient. Even so, scheduled patient may still have to wait depending on circumstances.
- Please be courteous with other patients and watch your kids all the time for their safety. There are toys provided to be played at your own risk. No cell phone or food allowed after entering the triage and exam room.
- For bounced check/ NSF, there will be additional fee charge.
- We do not accept any walk-in patient 30 minutes before break time or closing hour.
- If someone other than a legal guardian or parent accompanies your child, we must have a signed letter on file stating the relationship to the child and authorization for that person to approve any medical care that may be necessary.
- Prescription refills, referrals and appointments are handled only during office hours (Monday – Friday, 8am – 4.30pm).
- Please be advised that Medicaid/ Bayou Health has its own nurse line: the Medicaid Nurse First Advice line is a toll-free, round-the-clock number found on your Medicaid card.
- In our Lafayette Office, unfortunately at this time we are not accepting medicaid patients after 18 years old. We are sorry for any inconvenience.
